The medium quality videos step the resolution up to 400 x 300 pixels and increase the bitrate to over 600kbps. The highest quality WMV files are also at a resolution of 400 x 300 pixels, but the quality of the video is much better due to the videos being encoded at a much higher bitrate of 1.1Mbps or so.
